Documents needed for OOH/111 Onboarding

Never done an OOH/111 Shift in a particular Local Health Board? They'll need some documents to add you to their payroll. These can be uploaded into the 'Portfolio' Section of your profile for quicker/simpler onboarding

Once you've connected to a Local Health Board once, you generally don't need to do this again (unless it's been a significant period of time since working). Each Local Health Board has a slightly different check process and documents required for it.

Each Local Health Board will employ you directly. To onboard/induct/recruit quicker, if you upload the documents they'd like to view to your portfolio in advance it will save you and them time, and make it much simpler to work OOH shifts.

Aneurin Bevan University Health Board

Betsi Cadwaladr University Health Board

Cardiff & Vale University Health Board

  1. Pending receipt of onboarding information. Sorry we can't help you further here.

 

Cwm Taf Morgannwg University Health Board

Hywel Dda University Health Board

  1. Include in the Application Note: the Employee Number or something to confirm you're already inducted into the Health Board. If you haven't worked with Hywel Dda before, please note that.

Swansea Bay University Health Board

  1. Swansea Bay will require an excel completed to be able to onboard you 

  2. Please also include in the Application Note that you are familiar with Adastra, and if you have specific login for Swansea Bay.
